The Housing for Health Division of the Santa Cruz County Human Services Agency (H4H) was
created in November 2020 to bring together a coalition of partners and resources to prevent and
end homelessness within the County.
This website provides information about services for people experiencing homelessness. It also describes H4H’s services and activities.
