What is Smart Path, and what is the Smart Path Assessment?

Smart Path to Housing and Health is Santa Cruz County’s homeless coordinated entry system. If you are experiencing homelessness in Santa Cruz County and are looking for a permanent place to call home, the first step is to complete a Smart Path Assessment.

Smart Path prioritizes scarce housing resources based on medical and social vulnerability as determined by an assessment. By completing the assessment, you will potentially be considered for multiple housing assistance programs throughout the county. You will also be informed about how to access available non-housing resources, such as meals, showers, medical care, and government benefits.

Smart Path to Housing and Health is not a housing program or a guarantee of shelter or housing. Rather, Smart Path provides a process that seeks to enable the community to better utilize limited housing assistance resources. Once you complete a Smart Path Assessment, you will be contacted if you have been referred to a housing program.


How do I get a Smart Path Assessment?

In order to complete a Smart Path Assessment, please contact one of the Roaming Assessors listed below.
